General Medical Records Information

• Staff process requests in the order in which they are received.  We make every effort to quickly accommodate our urgent requests.

• Records are processed during business hours, 8am-4pm Monday through Thursday and 8am-3pm on Friday.

• If you choose to have your records on paper, it will be a cost of $.15 per page.

• Due to confidentiality laws, releases of medical records require a completed Release of Information.
