Your First Visit

It’s completely normal to feel nervous before your first visit but the fact that you are taking or even considering this step is mighty impressive, so give yourself  a firm pat on the back. During your initial visit you will be asked questions about your mental health and your general health.

The information you share at this meeting is considered confidential.

You will learn about NorthCare services and see how we can help you in addressing your concerns. You will leave with a plan for the upcoming week that will be tailored to your needs.

During your initial visit you will need to complete some paperwork, the first visit typically takes longer. So please be prepared to spend at least a few hours at NorthCare.

1.) Identification Card (state I.D. with picture)**

2.) Social Security Card**

3.) Insurance Card**

4.) Verification of income ( 1 of the following forms)

• Medicaid Card
• Two (2) recent, consecutive paycheck stubs (showing pay date, hours worked, type(s) of pay, and gross pay rate(s))
• Federal Income Tax Return
• Verification of inform from current employer
• Federal Form W-2(s) (if you have been employed for a full year on a full-time basis)
• Any government document that verifies income
• If none of the above documents are available, please bring an income statement signed by the client, family member, or facility staff member verifying lack of income.
